DIGITAL SIGNATURE FOR RELEASE WAIVER: As the parent or guardian of the child registering for the Just Jumpin’ jump rope workshop, I hereby release the Just Jumpin’ jump rope team and its affiliates, coaches, team members, employees, volunteers, and owners and lessors of premises used to conduct the jump rope workshop (collectively and hereafter referred to as the "Released Parties"), from any and all liability, claims, demands, and causes of action that I may have now or in the future, arising out of or related to any loss, illness, damage, or injury, including death, that may be sustained by me or my ward/child, or to any property belonging to me or my ward/child, whether caused by the negligence of the Released Parties or otherwise. I understand and acknowledge that my child’s/ward’s participation in the sports activities organized by Just Jumpin’ involve inherent risks, including but not limited to bodily injury, permanent disability, property damage, and death. My digital signature below signifies that I voluntarily assume all risks associated with my child’s/ward’s participation in these activities.
